How to Refer to the Diabetes Education Program
Self-Referrals:
No referral is necessary to see any of our Registered Nurses or Registered Dietitians . Anyone living with Type 2 Diabetes or Prediabetes is eligible to attend our programs regardless of where you live. Call : 416-787-1676 ext. 3303 to make your appointment.
Referrals from Doctors and Health Care Professionals:
If you are a health care professional and wish to refer your patients to our program, please complete the referral form below and fax the form to our team. Fax: 647-260-0310. Our receptionist will arrange an appointment for your patient. (Please complete the insulin order on page 2 if your patient requires insulin adjustments and attach the patient’s recent blood work.)

If you are interested to refer to our Teleophthalmology Program, please call 416-787-1661 ext. 3303 or ask your family doctor or nurse practitioner complete the referral form below and fax it to (416) 461-8245, Attention: Teleophthalmology Program.
