Providing Feedback

At Unison, we welcome input and feedback from clients about our programs and services – both on what we do well and what we need to improve. We are looking at some new and creative ways for you – our clients, our clients’ families and community members – to share your ideas and feedback to help us plan and improve.

As a client, some of the ways you can give us your feedback include:

  • Completing our Client Experience Survey conducted every two years.
  • Sending us a suggestion or complaint.
  • Completing an evaluation form when you attend a group program at Unison. We evaluate most of our group programs at least once a year.
  • Calling any Unison phone number and dialing ext. 3399
  • Electronically by completing our Feedback Form. Please note the form will be submitted to us using e-mail which is not a secure communication method. If you provide your e-mail on the complaint form, we will assume we have your permission to contact you by e-mail regarding the complaint.
  • In person by filling out a compliment/feedback card and putting it in the feedback box at your site.
