WestReach offers mental health and substance use counselling as well as chronic disease monitoring, weight management and nutrition education to residents of the Rockcliffe-Smythe and Mount Dennis neighborhoods. Our goal is to minimize the barriers that physicians face when referring clients for mental health and chronic disease services who would otherwise not be able to access these services. Referrals should be made by your family physician.
The program is comprised of an inter-professional (IP) team designed to meet the various important needs of the clients it serves. The team includes:
A Community Mental Health and Substance Use Outreach Worker who will provide support, extensive community engagement and resource navigation to physicians, clients and partner agencies.
A Community Mental Health and Addictions Worker who will provide mental health education, harm reduction, and counselling services
A Registered Social Worker who will provide assessment, treatment planning, client-centred interventions and counselling, client education and referrals
A Registered Nurse who will provide client education, chronic disease monitoring, nutritional education and liaising with primary care physicians for medication management
Services provided will include:
- 8-10 brief one to one and/group counselling services
- Co-ordinated care management
- Goal-focused plans based on the recovery and harm reduction models
- Extensive internal and external community referrals
