Our APSW team provides intensive case management and supportive counselling to adults with developmental disabilities who live independently in the community. Clients are referred to the program through central intake at Community Living Toronto. The majority of client appointments are conducted through home visits and arrangements to meet in the community.
Our team also manages and responds to client crises which vary from evictions to hospitalization, attempts to self harm and involvement with the court system. In addition to counseling and case management, we offer a social activity group and art program to complement the services.
Adult Protective Services include:
Supportive Independent Living (SIL) Workers provide more intensive life skills training and assistance to Adult Protective Service clients who need a high level of one-on-one support to live independently in the community. SIL Workers visit their clients several hours each week to reduce isolation, accompany clients on outings into the community and teach them how to shop, cook for themselves and learn new skills essential for daily living.
